Home and Community-Based Services (HCBS) help individuals who meet nursing facility level of care remain safely at home and in the community. HCBS supports participants in the least restrictive environment while improving independence, safety, and quality of life. Medicaid-funded HCBS may be provided through State Plan services or 1915(c) Home and Community-Based Waiver programs.
Eligibility (Simple)
You may qualify for HCBS services if you:
- Are enrolled in MO HealthNet (Missouri Medicaid)
- Meet the nursing facility’s level of care requirements through an assessment
- Need assistance with daily activities such as personal care, mobility, or safety supervision
