Consumer Directed Services (CDS) – Missouri Medicaid
Provision Home Care Inc. supports individuals enrolled in the Missouri Medicaid HCBS Consumer Directed Services (CDS) program. CDS allows eligible participants to receive care at home while having the freedom to choose their own caregiver, including a trusted family member or friend.
We help clients remain safe, independent, and comfortable in their own homes while ensuring compliance with state requirements.
Who is Eligible?
You may qualify for CDS if you:
- Are enrolled in MO HealthNet (Missouri Medicaid)
- Have a verified need for assistance with daily living activities
- Meet the program requirements for in-home services, including the ability to self-direct care
- Live in our service area (St. Louis City, St. Louis County, St. Charles County)
Eligibility is officially determined through your Medicaid plan and care coordination assessment.
Services May Include
- Assistance with bathing, grooming, and dressing
- Meal preparation and feeding assistance
- Toileting support
- Mobility/transfer assistance
- Light housekeeping related to health and safety
- Medication reminders (non-medical)
